To Register:
Call (253) 864-3330 between 9:00 a.m. and 4:00 p.m., Monday through Friday. Please provide item number and email address when registering.
You will not be allowed to attend classes without proper documentation. Please send your documents to:

Registration must be completed by February 5, 2010.
All documentation must be received in the CE office no later than February 12, 2010.

All documentation must be received in the CE office no later than July 16, 2010. NO REFUNDS will be issued after July 2, 2010.
Full payment for Nitrous Oxide and/or Local Anesthesia is due at registration.
A $550.00 non-refundable deposit is required when registering for the Restorative Dentistry course, with full payment due by July 2, 2010. NO REFUNDS will be issued after July 2, 2010.
