Application Process

APPLICATION DEADLINES: 

March Session: January 23, 2013

August Session:  June 15, 2013


A complete application packet must be submitted via hard copy by application deadline. Official transcripts should be mailed directly from the school. Students with incomplete packets will be wait-listed. Seats cannot be reserved.

Be prepared to register and pay the full course fee when you receive permission to register for the class.

Submit PNDHI application packet:

  1. Download and complete our PNDHI application form.

  2. Provide proof of licensure:  (choose one of the following)

    • Copy of current Washington state license. (temporary, expired, or permanent)
    • Copy of license (temporary, expired or permanent) from another state or Canadian province
    • Documents that prove licensure eligibility in Washington and for the WREB exam:
      • official transcripts from an ADA accredited dental hygiene program OR
      • certified (notarized) COPY of a certificate (diploma) OR
      • letter from the school on official letterhead indicating you will graduate from an ADA accredited dental hygiene program prior to the exam date

  3. Provide proof of current immunity status.

    • Copy of a Hepatitis B vaccination OR proof of immunity confirmed by a titer test
    • Copy of TB antigen test current within one year or Physician’s report.

  4. Provide proof of current Healthcare Providers CPR and First Aid certification

Please Note: Canadian students must submit the equivalent documents from their school and/or College of Dental Hygienists.

Students who are not U. S. citizens must first get an International Student ID number before submitting a registration form. Please call Jen Gruver, International Education Activities and Admissions Coordinator at (253) 964-6656 or email her at jgruver@pierce.ctc.edu to be assigned an International Student ID.

Acceptance and Registration:
Students accepted to the PNDHI will be notified and provided with a registration and payment link.  Students should be prepared to register and pay in full when applying to the program. Pierce College accepts VISA, MasterCard debit or credit cards and personal check.
